Please read the following carefully before signing this application:

I understand that this is an application for and not a commitment or promise of volunteer opportunity. I certify that I have and will provide information throughout the selection process, including on this application for a volunteer position and in interviews with Catholic Welfare Services (CWS) and/or its institutions that is true, correct and complete to the best of my knowledge. I certify that I have and will answer all questions to the best of my ability and that I have not and will not withhold any information that would unfavourably affect my application for a volunteer position. I understand that information contained on my application may be verified by CWS. Further, I understand that misrepresentations or omissions may be cause for my immediate rejection as an applicant for a volunteer position with CWS or my termination as a volunteer.

Privacy statement (Provisions under the Personal Data Protection Act 2012):

  1. The personal information on this form is being collected for the purposes of recruiting and selecting volunteers wishing to work with Catholic Welfare Services. The information may also be required for evaluation purposes. Any evaluation reports developed will not identify individual volunteers by name. This information may be shared with the organisation’s Management Committee and funding bodies. Upon acceptance as a volunteer, I will consent to retention and use of my personal information for:
    • Maintaining of my volunteer relationship with Catholic Welfare Services;
    • Communication purpose;
    • Disclosure to 3rd parties for matters related to my volunteering work at Catholic Welfare
      • Services.
  2. As I will have access to the identities of CWS clients and other information, I shall keep all information fully confidential (including pictures and videos of clients or events, etc.) and will not collect, use or disclose them without permission of Catholic Welfare Services.

Minimum age requirement of CWS volunteer applicant
